We provide peer-led wraparound support services and mobile outreach that creates accessible pathways to improved health and wellness and sustainable housing.

Our 4-Step Peer Support Model

  • 1. TAKING IT TO THE STREET MOBILE OUTREACH

Streetscape's first peer-led program, Taking it to the Streets Mobile Outreach, is a low-barrier, community-centered street outreach. It leverages community trust to connect individuals experiencing homelessness with health and wellness services, housing resources, and essential supplies (including food and a twice-yearly pet care clinic). It also serves as a critical hub for peers to meet, stay in touch, and coordinate time-sensitive appointments.

  • 2. TWO-WAY STREET

Two-Way Street is Streetscape's second-step, voluntary peer support program for individuals experiencing homelessness. Enrollment requires consistent engagement with our peer services. We build peer-to-peer relationships, help participants obtain vital documents such as state IDs, provide information on job opportunities, help to maintain appointments for mental health services or housing, and provide access to essential, person-centered resources.

  • 3. STREETSMART

STREETSCAPE's third peer support program, Streetsmart, is designed for individuals progressing in their housing journey while still experiencing homelessness.

Streetsmart program participants voluntarily engage in a more intensive peer support model, which guides those in need in navigating the complexities of subsidized housing. Peers assist in obtaining necessary documents and signed releases of information, aid in apartment searches, and offer trusted peer advocacy. Through shared experiences, Streetsmart provides a supportive and understanding ear throughout the sometimes lengthy and confusing housing process.

  • 4. BEAT THE STREETS

🏘️ Beat the Streets: Housing Aftercare

Beat the Streets is Streetscape's fourth peer support program, offering intensive housing aftercare peer support to formerly homeless individuals and low-income families who have secured stable housing. Enrollees voluntarily engage in person-centered wrap-around peer support via one-on-one sessions and small groups, focusing on daily living adjustments and pro-social supports.

The program connects participants to vital resources like workforce development, job opportunities, rental assistance, food banks, legal services, and government programs (e.g., LEAP utility support). We also provide basic household essentials for the kitchen, bathroom, and bedroom to help make their homes functional and comfortable.
