Streetscape Programs

1. Coordinated Entry & E-Navigation Services

Streetscape provides immediate access to Coordinated Entry, housing assessments, and resource referrals. Partnering with 211 and the Metro Denver Homeless Initiative (MDHI), our e-navigation specialists help individuals experiencing homelessness access service referals from our regional Homeless Management Information System (HMIS, addiction recovery pipelines, and housing resources to secure stable placements as quickly as possible.

2. Community Outreach & Peer Navigation Events

Streetscape coordinates high-impact special-event outreach across Boulder County. These events provide immediate access to necessities while offering low-barrier, in-person resource navigation—including direct connections to addiction recovery programs. Our outreach is primarily peer-led by individuals with lived experience, building trust with those we serve. Streetscape outreach events are designed specifically for individuals experiencing homelessness, individuals who have transitioned out of homelessness and into stable housing, and other low-income families.

3. Transition & Continuity Support

Streetscape offers ongoing peer mentorship and person-centered resource connection for individuals transitioning out of homelessness. This program offers ongoing support for people navigating existing recovery frameworks and work-release programs, and for those who have acquired stable housing, by helping them stay accountable and build a community to maintain long-term stability.
